ATHLETIC REQUIREMENTS & FEES

General Participation Fees

(one time per school year)

  • $75.00 per child

  • $125.000 maximum per family

Required Athletic Forms

​

Athletic physicals and the following forms need to be completed and returned to the nurse's office prior to the start of the sport.

  • Athletic Physical Form

  • Required Medical Treatment Form

  • Code of conduct

  • Concussion Consent Form
